The following
is a list forms available to download in PDF format using
the Adobe Acrobat Reader. If you don't have this on your
computer a free download is available just click below:
This
form should be completed and sent to the convention chairman,
(See address list above) This will insure that future information
will be sent to the proper people. It will also allow us to
provide an updated directory for clubs to mail newsletters.
And please include your Hockey team’s phone number and, if
known, a contact for group tickets.
This file is copy of the constitution and by
laws as they were revised and approved at the
2006 convention in Philadelphia for reference by
Clubs and members.
This is the traditional first letter of
convention stating all of the pricing, due
dates, theme and extra night pricing. Read this
letter carefully as some things have changed.
This is the convention hosting order for
upcoming years. Many have requested this list
and the officers reviewed past minutes to make
sure this is how the list was revised with the
addition of many new clubs from 1998 to present.
You will receive an appropriate amount of registration
forms based on past attendance of your club. These are the
three part form that you are used to in the past. One is to
be completed for EACH ROOM. Please remind attendees to accurately
complete the ROOM INFORMATION block in the upper right corner
as well as circling your arrival day. The arrival day is very
important to coordinate room holds and hospitality at the
hotel. Also there are only 4 standard handicapped rooms available,
so I ask that only those who need these room types request
them. They will be assigned on a first received and required
basis. If you request a handicapped room and one cannot be
provided you will get a phone call from me.
Remember
you must make copies to send to both the
Treasurer and Convention Chairman. You can also
request more tri color forms to be mailed to
your club if needed. by contacting the
Convention Chairman listed above.
Deposit and Balance Forms:
Here are copies of the traditional deposit form
and new final balance form to mailed to both the Treasurer
and Convention Chairman when the appropriate payments are
sent. Mail them to the appropriate addresses listed below.
